What in the LDL Hell? A Cholesterol Deep Dive


I got a lab report back with five different cholesterol numbers and one vague sense of dread. Turns out “cholesterol” isn’t one thing, and treating it like one number hides most of the useful information.

The main players

LDL carries cholesterol to your tissues and gets blamed for buildup in artery walls. HDL carries it back to the liver for clearance, the “helpful” direction. Triglycerides are a separate fat measure entirely, more tied to sugar and alcohol intake than people expect.

The number that actually matters more

LDL particle count, often estimated through ApoB, turns out to predict cardiovascular risk better than plain LDL cholesterol. Two people can share an LDL number and carry very different risk depending on particle size and count.

Where I land

A single “high LDL” flag on a basic panel isn’t a verdict, it’s a prompt to look closer: the ratio to HDL, triglycerides, family history, and ideally ApoB if you can get it. Cholesterol is a system, not a scoreboard.
